What does it mean when a file is a 'ISO image, not exe'?

It means that the file is an "image" file of a CD, which is intended to be recorded onto a writable CD. So you can't use it directly ("run" it). There are programmes which will let you access the files inside the .iso image.

If you have trouble getting your head around that, then imagine that an ISO is like a "photo negative" of a CD.
Can't do much with it except produce another "photo". Thats all the ISO image is.

use cd creator to create a cd from it or alcohol 120% will mount it in a virtual drive and play it for you
