Not quite sure which programs I need to run at startup. Probably not too many. Which ones can I delete from startup please?.

I would uncheck everything except for ESET and Audio and uninstall Mcafee

Have the same problem at start-up with XP SP3, screen icons take forever to load, had unticked quite a few in Start-up.

Uninstall the Ask toolbar whatever it is. Untick (or delete the entries in startup with ccleaner), Java, QuickTime, ask, the canon entry, Adobe reader and Spybot. Uninstall powerreg, Mcafee.
