If a graphics card is not recognized in the device manager at all
does this indicate a faulty motherboard?

Might be the card try it in another computer. Might be the slot its in. Try another slot, if there's another slot

Is it Onboard? Or PCI-e / AGP? What's the make / model of the card?

You may need to update the motherboard chip set drivers...

If the computer is working with it in, then it is more likely to be a driver issue. Look for an entry under Other or Unknown, if you have such an entry in your device manager, download and install the drivers, if successful, it should then be listed under Display Adaptors. What sort of graphics card is it?
