what would cause a incandescent down-light bulb to explode??

had one do just that this morning, it was on at the time

How old was the light? - might have been an imperfection in the glass...

it was one of those flat mushroom type bulbs

Ah! There's your problem - mushrooms are supposed to be kept in the dark. I bet you lit this one up!!! :D

I could see that happening if a crack developed in the glass, allowing oxygen in. Quite entertaining to watch a filament go when exposed to air - as opposed to the inert gas that should surround it.
And we're on 230v, the real deal.

It could be worse.

Some people have letterboxes that explode!
